Join us on Tuesday, February 20, 2024 at 11AM to 12:30PM for a FREE lunch & learn seminar where members of the community can come together to learn more and ask questions about Advance Planning and recording end-of-life decisions at San Tan Memorial Gardens at Schnepf Farms.
We will be joined by Hospice of the Valley who will introduce Choose Your Person: a statewide campaign urging Arizonans choose the person they want to advocate for them and carry out their healthcare wishes.
Specially trained community members will empower attendees to discuss their wishes with loved ones; record their preferences by completing Arizona’s healthcare directive forms; and then store the documents safely in the registry, accessible to family and healthcare providers alike, for those critical moments when they are needed.
